About the Program

Integrating health care delivery to facilitate access to comprehensive care programs that address both the behavioral health and medical needs of patients in one integrated setting is a focus of health care transformation at the state and federal level. Historically, behavioral health and primary care services operated separately, situated apart from one another, in different physical locations, with different regulatory oversight and program certification, separate medical records, providers and care teams, and billing systems. Patients with both medical and behavioral health conditions had to negotiate the separate bureaucracies often, and not surprisingly, with limited success. Patients in primary care suffered with untreated mental health conditions and patients in mental health clinics had deteriorating chronic medical conditions which remained medically untreated. The goal for integrating behavioral health and primary care was to address these inequities and again recognize that the patient may benefit from a “holistic approach” to care.

Implementing a successful transformation however has been proving to be challenging and at times elusive. In this session, our first guest speaker, Dr. Andrew Cleek, who has worked with behavioral health clinics throughout New York, will discuss how the evidence based guidelines to drive practice management transformation has helped to evolve community based behavioral health clinics and implement co-located behavioral health and primary care services.

In the second part of this session, Patricia Belair and Dr. Lizica Troneci, SBH Health System in the Bronx, will discuss how they use integration models as their directional paradigms to redesign services and establish integrated practices. Their presentations will focus on what new and reconfigured existing resources have assisted their health system with addressing specific operational, technical, and culture change challenges created by the integration of these previously separated care delivery silos.
About the Speakers

Andrew F. Cleek (PsyD)

Dr. Cleek oversees community partnerships and co-directs both New York State’s Community and Managed Care Technical Assistance Centers (CTAC and MCTAC) in his role as the McSilver Institute’s Executive Officer. In this role Dr. Cleek has overseen the development of a NYS funded technical assistance platform that provides support to all Mental Health, Substance Use, and Child Welfare Providers in NYS. Dr. Cleek was the founding director of the Urban Institute for Behavioral Health (UIBH), which was a unique partnership of behavioral health providers dedicated to the implementation of evidence-based practices, integrated primary and behavioral healthcare, and system transformation.

Patricia S. Belair

Patricia received her BS in Nursing from Russell Sage College and her MA in Nursing Administration/Community Health,from New York University. She began her nursing career as a community health nurse with the Visiting Nurse Association of Albany then in New York City with the Visiting Nurse Service of NY. After receiving her Master’s degree she joined Group Health Incorporated. From 1991-1997 she held the title of Corporate Officer/Senior Vice President Health Care Services.Responsibilities included oversight of a Certified Home Health Agency, Article 28 Dental Practice and an HRA personal care program.

Lizica C. Troneci, M.D. Chair, Department of Psychiatry at SBH Health System, is a general psychiatrist with 14 years of clinical experience and 8 years of administrative experience who has held the current position since October 2014. Prior to this she was the Vice-Chair of the Department of Psychiatry at Bronx Lebanon Hospital Center and has been a Psychiatry Residency Program Director for 8 years at the same institution.

As SBH Department Chair she oversees the administrative and clinical aspects in all settings: the inpatient division with a capacity of 49 adult beds; the outpatient behavioral health division servicing approximately 3,900 patients; and a newly developed collaborative care division in the ambulatory outpatient clinic.
